I understand to do so would result in my being eliminated from further consideration for employment. I further understand that, if accepted for employment, any misrepresentation or material omission may result in immediate termination of my employment, regardless of the time elapsed since my employment. I also authorize my former employers, educational institutions, Federal, State and/or local agencies and any other association or institution having control of any documents, records and other information pertaining to me to disclose any and all information requested, without giving me prior notice of such disclosure.
